Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver targeted physical therapy interventions addressing mobility, strength, coordination, and functional independence
• Partner with a multidisciplinary care team — including physicians, nurses, case managers, and fellow rehab specialists — to drive optimal patient outcomes
• Provide meaningful education to patients and their families around therapeutic goals, home exercise programming, and discharge preparation
• Maintain accurate, timely clinical documentation aligned with facility protocols and regulatory standards
• Actively contribute to interdisciplinary rounds and care conferences to ensure a seamless, coordinated continuum of care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in Texas
• CPR certification preferred
• Specialty certifications such as neurological or geriatric rehab are welcomed, though not required
