Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-driven treatment plans
• Deliver evidence-based physical therapy interventions targeting strength, mobility, balance, coordination, and functional independence
• Partner with physicians, nursing staff, case managers, and fellow rehabilitation specialists to deliver coordinated, patient-centered care
• Guide patients and their families through therapeutic goals, home exercise programming, and discharge readiness
• Maintain accurate, timely documentation of assessments, interventions, and patient progress in alignment with facility and regulatory standards
• Actively contribute to interdisciplinary care conferences and team rounds to support seamless transitions across the continuum of care

Education

• Doctorate in Physical Therapy (DPT) or equivalent accredited degree required

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, orthopedics, or neurology are welcomed but not mandatory

Skills

• Prior clinical experience is strongly preferred, though passionate new graduates are encouraged to apply
