Description

• Conduct evaluations in inpatient or outpatient settings and craft personalized treatment plans tailored to each patient
• Implement physical therapy interventions that target mobility, strength, balance, and functionality
• Collaborate effectively with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
• Provide education to patients and families regarding therapeutic goals, discharge strategies, and home exercise programs
• Document patient assessments, treatments, and progress in compliance with facility and regulatory standards
• Engage in care team rounds and interdisciplinary conferences to ensure seamless, coordinated care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Valid state license as a Physical Therapist
• CPR certification is preferred
