Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver targeted physical therapy interventions addressing mobility, strength, coordination, balance, and overall functional capacity
• Partner closely with physicians, nursing staff, case managers, and fellow rehabilitation professionals to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, home exercise programming, and discharge preparation
• Maintain accurate and timely documentation of evaluations, sessions, and patient progress in compliance with facility and regulatory standards
• Actively contribute to interdisciplinary care conferences and team rounds to ensure seamless, coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or equivalent accredited degree

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Advanced or specialty certifications (e.g., neurological rehabilitation, geriatrics, orthopedics) are welcomed but not required
