Description

• Perform thorough patient evaluations in inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver targeted physical therapy interventions designed to improve mobility, strength, coordination, and functional independence
• Work alongside physicians, nurses, case managers, and fellow rehabilitation specialists to deliver seamless, patient-centered care
• Guide patients and their support systems through therapeutic goals, home exercise programs, and discharge preparation
• Maintain detailed, accurate clinical documentation in compliance with facility policies and applicable regulations
• Actively participate in interdisciplinary rounds and care conferences to ensure holistic, coordinated outcomes

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Specialty certifications in areas such as neurology or geriatrics are welcomed but not required
