Description

• Conduct comprehensive patient evaluations in inpatient or outpatient settings and build individualized, goal-driven treatment plans
• Deliver physical therapy interventions targeting mobility, functional strength, coordination, and balance restoration
• Work hand-in-hand with physicians, nurses, case managers, and fellow rehab professionals to ensure cohesive, patient-centered care
• Guide patients and their families through therapeutic goals, home exercise programs, and discharge preparation
• Maintain accurate and thorough documentation of evaluations, sessions, and patient progress in compliance with facility and regulatory standards
• Engage actively in interdisciplinary rounds and care conferences to support seamless transitions across the continuum of care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not required

Skills

• Prior clinical experience is strongly preferred
• Demonstrated ability to manage patients presenting with complex medical and functional challenges
• Excellent interpersonal and communication skills with a collaborative mindset
• Proficiency with electronic health records and digital documentation platforms
