Description

• Perform comprehensive patient evaluations across inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver physical therapy interventions targeting mobility restoration, functional strength, postural balance, and overall physical performance
• Engage in collaborative practice alongside physicians, nursing staff, case managers, and fellow rehabilitation specialists to drive optimal patient outcomes
• Provide thorough education to patients and their families regarding therapeutic objectives, home exercise programs, and safe discharge transitions
• Maintain accurate, timely clinical documentation in compliance with facility protocols and state and federal regulatory standards
• Contribute actively to interdisciplinary team rounds and care conferences to ensure a seamless, coordinated continuum of care

Education

• Doctor of Physical Therapy (DPT) or equivalent graduate degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Advanced or specialty certifications such as geriatric, orthopedic, or neurological focus are welcomed but not required
