Description

• Conduct comprehensive patient evaluations in inpatient and/or outpatient settings and develop individualized, goal-driven treatment plans
• Deliver evidence-based physical therapy interventions targeting mobility restoration, muscular strength, balance, and functional independence
• Work alongside nurses, attending physicians, case managers, and fellow rehabilitation specialists to deliver coordinated, patient-centered care
• Guide patients and their support systems through therapeutic goals, home program development, and discharge planning
• Maintain thorough, timely documentation of evaluations, sessions, and patient progress in compliance with facility protocols and regulatory standards
• Engage actively in interdisciplinary rounds and care conferences to ensure continuity and quality across the care continuum

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a regionally accredited institution

Certifications

• Active Physical Therapist licensure in the state of Utah (or eligibility to obtain)
• CPR certification preferred
• Advanced specialty certifications such as neurological or geriatric rehab are welcomed, though not required

Skills

• Prior clinical experience is strongly preferred
• Demonstrated ability to manage patients presenting with complex medical and functional challenges
