Role Responsibilities

Education Requirements

• MD or DO from a recognized medical institution.
• Completion of a family medicine residency accredited by the ACGME.

Certifications

• Board Certified in Family Medicine.
• Current, or eligible for, an active medical license in the state of Utah.
• Preferred certifications in BLS and ACLS.
