Description

• Conduct thorough patient evaluations in both inpatient and outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver physical therapy interventions targeting mobility restoration, strength development, balance training, and functional independence
• Work closely alongside physicians, nursing staff, case managers, and fellow rehabilitation professionals to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, discharge preparation, and home exercise programming
• Maintain accurate and timely documentation of evaluations, ongoing treatments, and patient progress in accordance with facility and regulatory standards
• Actively engage in interdisciplinary rounds and care conferences to support a fully coordinated approach to patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree earned from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Specialty certifications such as neurological or geriatric rehabilitation are welcomed but not required

Skills

• Prior clinical experience is strongly preferred
• Demonstrated ability to manage patients presenting with complex medical and mobility challenges
