Job Summary

As an Integrated Behavioral Health provider, you will conduct initial patient evaluations, triage assessments, crisis intervention, brief counseling, skills training, care coordination, case management, advocacy, and follow-up. Collaboration with primary care providers and participation in interdisciplinary case conferences and program development are key components of this position.

Key Responsibilities

• Perform timely behavioral health assessments, diagnoses, and treatment planning, utilizing data from patient records, interviews, and observations.
• Maintain accurate and complete documentation of patient progress in electronic medical records.
• Work closely with the integrated primary care team to ensure coordinated, patient-centered care.
• Provide immediate crisis intervention and behavioral health support during face-to-face patient encounters.
• Serve as an advocate and resource for patients, families, care teams, payers, and community resources, fostering effective communication and support.
• Participate actively in committees, lead projects, and contribute to continuous improvement initiatives.
• Ensure compliance with legal, accreditation, and third-party payer documentation standards.
• Engage in community outreach and supervise students or residents as needed.

Education and Experience

• Master’s or doctoral degree in Social Work or Mental Health Counseling from an accredited institution.

Licenses and Certifications

• Unrestricted LMFT, LICSW, or LMHC license valid in the state of Washington.
• Current Basic Life Support (BLS) certification.
