Job Summary

As a primary care provider, you will manage a dedicated panel of patients, offering first-contact and ongoing care aligned with the patient-centered medical home model. You will collaborate closely with other healthcare professionals to ensure evidence-based, high-quality treatment and positive patient experiences.

Key Responsibilities

• Deliver family medicine services to clinic and hospital patients requiring primary care.
• Engage in quality improvement and risk management initiatives related to family medicine.
• Participate in policy development and implementation affecting practitioner services.
• Maintain accurate and timely medical records in compliance with legal and accreditation standards.
• Fulfill on-call and call coverage duties as required by medical staff bylaws.
• Contribute to community outreach efforts and support patient and provider satisfaction initiatives.
• Assist with supervision and provide input on staffing, budgeting, and equipment needs.
• Adapt to changes, foster teamwork, and uphold professional and ethical standards.

Education and Experience

• Successful completion of an ACGME or AOA accredited residency program.
• Board certification or eligibility in Family Medicine.

Licenses and Certifications

• Active, unrestricted MD or DO license valid in the relevant state.
• DEA certification.
• Current Advanced Cardiac Life Support (ACLS) certification.
