Description

• Conduct evaluations in both inpatient and outpatient realms, crafting personalized treatment plans for patients
• Implement physical therapy interventions aimed at improving mobility, strength, balance, and overall function
• Work alongside nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Guide patients and their families through therapeutic goals, discharge preparations, and home exercise regimens
• Maintain documentation of evaluations, treatments, and progress in alignment with facility standards and regulations
• Engage in collaborative care team rounds and interdisciplinary meetings to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
