Description

• Assess patients in either inpatient or outpatient environments, tailoring individualized treatment plans to meet diverse needs
• Implement physical therapy interventions that enhance mobility, strength, balance, and overall functional capacity
• Partner with nurses, physicians, case managers, and other rehabilitation professionals to ensure the best possible outcomes
• Guide and educate patients and their families on therapeutic goals, discharge plans, and at-home exercises
• Maintain thorough documentation of evaluations, treatments, and progress aligned with facility policies and regulations
• Engage in multidisciplinary care team rounds and conferences for coordinated and comprehensive patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent credential from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred
• Specialty certifications (e.g., neurological, geriatric) are advantageous but not mandatory

• Prior experience strongly preferred
