Description

• Conduct thorough patient evaluations in inpatient or outpatient settings and develop personalized, goal-driven treatment plans
• Deliver targeted physical therapy interventions addressing mobility, strength, coordination, balance, and overall functional ability
• Partner closely with physicians, nurses, case managers, and fellow rehabilitation professionals to drive optimal patient outcomes
• Provide clear, compassionate education to patients and their families regarding therapeutic goals, home exercise programs, and discharge planning
• Maintain accurate and timely documentation of evaluations, session notes, and patient progress in alignment with facility standards and regulatory requirements
• Actively participate in interdisciplinary rounds and care conferences to ensure seamless, coordinated treatment across the care continuum

Education

• Doctorate in Physical Therapy (DPT) or equivalent accredited degree

Certifications

• Active state licensure as a Physical Therapist in Wisconsin
• CPR certification preferred
• Specialty certifications such as neurological rehab, geriatrics, or orthopedics are welcomed but not required

Skills

• Prior clinical experience strongly preferred
• Demonstrated ability to manage patients with complex medical histories and multifaceted mobility challenges
• Exceptional interpersonal and communication skills with both patients and care team members
• Proficiency with electronic health records and digital clinical documentation platforms
